LG Polymers GAS Leak is a Human Error: FSL

Hyderabad: As per the FSL sources, the styrene leakage occurred in LG Polymers at Vishakhapatnam on May 07th, due to human error and negligence in maintenance during lockdown.

This mishap occurred at the time of scoured the LG Polymers site in Vishakhapatnam for clues. A team led by Dr. R K Sarin and T. Suresh of AP Forensic Science Laboratory visited the gas tragedy site and collected evidence over the past two days. Human negligence in adding auto polymerization inhibitor in the styrene storage tank and failing to maintain the temperature below 20 degree centigrade during the lockdown period led to the mishap that claimed 12 lives, said the preliminary report of the AP FSL.

The experts said that Styrene should be mixed with territory butyl, catechol (TBC), to prevent self-polarization. During the lockdown period the TBC was not added and whatever was existing got inactive.
